During our survey activity for this exposure, we were sent to our community from barangay Poblacion via a pick-up truck along with Tuboran. Our survey activity lasted for about 10 days. The survey questionnaire was also standardized, so all of the teams have the same data.
We collated everything but tallied only the chosen questions that has a big impact on the community. We came up with 5 Community Health Plans; Constraint to Potable Water Supply, Insufficiencies of Provisions at the Barangay Health Center, High Prevalence of Hypertension, Unsanitary Toilet Condition, and Solid Waste Management.
Other factors for the CHPs were the Focused Group Discussions that were conducted in each sectors (Farmers, Women, Youth, Senior Citizens), and their prioritization.

Data obtained related to Constraint to Potable Water Supply
Figure 1: Drinking Water Source: 66% of the surveyed residents responded to tabay as the number one source of drinking water
Figure 2: Distance from Household to Water Source: 19% of the respondents’ household is more than 150 meters to the water source which cued to the problem
Figure 3: Method of Water Sterilization: 78% of the surveyed residents don’t practice any water sterilization techniques
For Prevalence of Hypertension, the following data were obtained.

Data regarding Insufficiencies of Provisions at the Barangay Health Center are the following:
110 pregnancies surveyed by the team didn’t have any prenatal check-ups. The age of pregnancies surveyed ranges from 16 to 46 years old.
Figure 5: No prenatal check-ups: 65% of the respondents stated that prenatal check-ups were not yet implemented when they had their pregnancy. Some of the reasons why women don’t consult for prenatal check-ups are due to the distance, lack of services offered, and lack of money

Data obtained from the survey regarding Unsanitary Toilet Condition:
Figure 9: Toilet Condition: Upon survey and observing their toilet with a given criteria, 60 households have good toilet condition 42 households were fair and 14 households have poor toilet condition.
Figure 10: Comfort Room Location: Majority of the household have their toilet outside their house and 2 households practices open defecation in the river.
Data obtained for Solid Waste Management
Figure 11: Waste Disposal: Composting is the main mode of disposal of the residents in Barangay Balanan with 50% and followed by open burning with 30%.
Figure 12: Proper Segregation: 67% of the surveyed households practices proper segregation of waste. 33% of them lacks knowledge and responsibility of proper segregation.

Figure 13: Prioritization by the Barangay Council: Water ranked number 1 as their problem followed by Road Development.
Table 3: Top Morbidity in Balanan for the year 2016: The top 3 are upper respiratory tract infection with 40 cases, urinary tract infection with 36 cases and skin diseases with 28 cases.
Table 4: Top Mortality in Balanan for the year 2016: The causes of mortality includes hypertension and injury with 2 cases each and diabetes mellitus and pulmonary tuberculosis with 1 case each.
